this form must be filled in to sign up for AW Dance Academy dance classes.

My child has a medical condition
My child has a learning difficulties

You only need to answer this question if you child attends an after school club. I give permission for AW Dance Academy to access and give medication stored at my child's school.
The Parents/Guardians of the child stated on this form feels their child is in good health to participate in dance. If any of the details declared above changes they will make AW Dance Academy aware
